An exciting opportunity for a mental health nurse has arisen to join our innovative Community Mental Health Team based in Stoke on Trent. You will be based at the Greenfield Centre in Tunstall, and the working hours are Monday to Friday from 9am to 5pm, although part-time hours will be considered.
You will be expected to be highly motivated, enthusiastic, with excellent communication skills, and capable of working independently within a small team. If successful, you will be working to a community locality working model with GPs, other mental health services, and the wider primary care teams across the area. Flexible working will be considered.
You will need to have a core RMN qualification.
Our community mental health team is made up of a vibrant and supportive multi-disciplinary team. We aim to support our staff development with lots of opportunities to develop your skills and learning. There will be opportunities to lead on projects, support research, and quality improvements. This is an exciting time for our team as we continuously develop more effective therapy-based pathways, with a focus on recovery for our service users.
The successful applicant will work alongside our current mental health nurses and occupational therapists to develop these group and individual recovery-focused pathways.
Main duties of the job
* To work as a member of the CMHT, providing a service to clients and their families.
* The post holder will be responsible for the assessment of care needs, the development, implementation, and evaluation of recovery-based care programmes, specifically evidence-based care pathways.
* To act as care co-ordinator.
* To work alongside a multi-disciplinary team.
* To work in conjunction with other providers from statutory and non-statutory services.
* To provide supervision and support to junior staff members assisting in the caregiving process.
* To contribute to the future development of care services.
* To contribute their core professional knowledge and skills to the multi-disciplinary team and to uphold professional standards and ensure high standards of quality clinical care.

Job responsibilities
* To assume care co-ordinator responsibility for a caseload of patients and to ensure that each has a current, individualized care plan based upon assessed need.
* To carry out all therapeutic activities and associated tasks for patients within the clinical area without direct supervision.
* Supervise junior/unqualified staff in the provision of care.
* To contribute to the day-to-day operation of the clinical area in respect of delegated administrative procedural tasks.
* To support senior staff in the planning and implementing of new initiatives.
Person Specification
Qualifications
* A qualified/registered mental health nurse.
* Evidence of professional updating and development in the delivery services to people with mental illness.
Knowledge
* Knowledge of various approaches to mental health assessment.
* Awareness of harassment, social exclusion issues, and the effects of personal and institutional discrimination.
Experience
* Knowledge of adult community nursing.
