To undertake the role of peer support worker for an allocated caseload of service users, organising and participating in the ongoing assessment, planning, delivering, and review of care to meet identified physical and mental health, social, and wellbeing needs. This will include: Explaining the role of peer support worker Organising and participating in defined assessment methods and activities Contributing to care plans and identifying other resources as appropriate Completing relevant paper and electronic documentation in line with the peer support worker role Developing, planning, and undertaking defined interventions to enable the individual to meet and develop their ongoing daily living skills and increase confidence and independence in line with agreed personal recovery plans. Defined Self-Help Groups Preparing for and delivering defined self-help groups Conducting therapeutic activities to improve coping skills, e.g., mindfulness and relaxation Co-facilitating therapeutic group work Supporting Daily Living Skills Planning and undertaking activities to enable the individual to meet and develop their daily living skills, increasing confidence and independence in line with agreed personal recovery plans. This might include helping individuals to: Complete forms or letters, e.g., housing benefits Contact employment advisors or education facilities Emotional and Practical Support Providing emotional support focused on motivation and encouragement Offering information and advice to maintain good physical health Providing information on rights under the mental health act Effectively build hope, inspire relationships that acknowledge each person’s personal journey, focusing on strengths and aspirations to create meaningful personal recovery plans. Work collaboratively with service users and their carers to understand and manage their mental health needs in line with their personal recovery plan and relapse prevention strategies. Supporting Carers Undertaking specific activities and interventions to support carers, including engagement, involvement, assessment, and provision of support or services. Participate in the development of risk assessments and crisis management plans, ensuring appropriate reporting and action in response to crises. Respond to the needs of individuals with sensitivity regarding their age, culture, race, gender, ethnicity, social class, or disability, and promote the rights of individuals by recognising differences and acting in accordance with relevant legislation. Developing Skills Work collaboratively and sensitively with individuals to develop skills to manage their own health, actively promoting approaches that build on strengths and past positive experiences, using small steps to achieve goals. Independence and Confidence Promoting independence and increasing confidence by providing self-help materials, information about available resources, facilitating group work, educational workshops, and peer support groups. Be responsible for maintaining a caseload, prioritising time effectively, and contributing to the protection of individuals from abuse and harm in line with local safeguarding policies. Maintaining Records Maintain accurate, confidential records of patient activity, writing reports and letters, and utilising electronic systems available within the organisation. Report and record activity within agreed timeframes to meet information reporting and performance requirements. Participate in local arrangements to ensure consistent care to service users across the local geography.
