Hours: 37.5 hours per week (flexible options available). Monday to Friday, 9am-5pm (we're happy to consider condensed hours).
Main Duties At the Stockport Access Team, we're here to make sure people receive the right care, at the right time, from the right service. Our work involves:
* Screening and triaging all incoming referrals.
* Conducting telephone triages and urgent or routine face-to-face assessments.
* Guiding individuals to the most appropriate care pathways.
Your Role In this role, you'll play a key part in helping individuals access the mental health and social care support they need. Here's what you'll be doing:
* Carrying out thorough assessments of the health and social care needs of referred individuals, including detailed risk evaluations.
* Working within a multidisciplinary team to effectively respond to referrals from GPs and other agencies in the primary care setting.
* Providing short-term support, crisis management, and harm minimisation for individuals experiencing mental health challenges.
Key Responsibilities
* Assess and manage risk, signposting or referring individuals to the appropriate mental health services.
* Build effective links between the Access Team and other mental health services, helping to shape the team's gateway and assessment function.
* Offer urgent assessments for individuals presenting with acute mental health needs through the urgent care pathway. You'll identify care plans, follow-up needs, and make referrals to specialist services when required.
* Deliver brief crisis interventions and follow-up appointments to support individuals in managing crises.
As part of this role, you'll work in line with Pennine Care NHS Foundation Trust policies, the Mental Health Act, and community care legislation to ensure high-quality, compassionate care.
