Senior Care Assistant - Bridging team
Salary: £14.00 per hour plus 45p per mile - paid in block from start to finish of each shift including all travel time
Shifts: 8am-2pm, 4pm-10pm. You must be available to work alternate weekends.
Areas: This role covers areas including Benfleet, Basildon, Canvey Island, Wickford, Rayleigh, Chelmsford, Maldon, Braintree, and Witham.

Pay and shift patterns:
* Daytime shifts 8am-2pm, evening shifts 4pm-10pm.

About the role:
MSE Bridging covers Mid and South East Essex and offers a short term interim service for individuals being discharged from hospital, who are awaiting provision with either reablement or long term domiciliary care.
The Bridging Service provides support anywhere from 1 day to 2 weeks, while further care is being sourced.
Responsibilities Include:
* To support the hospitals in south, mid and north Essex with safe patient discharge care plans.
* To support customers with all aspects of their day to day living, so they can enjoy the best possible quality of life.
* Complete initial risk assessment and medication risk assessment, uploading discharge documentation.
* Report any changes or causes for concern to your line manager promptly.
* Regular feedback and communication with office.
* To record customer progress against their desired individual outcomes and provide pro-active feedback.
* Ability to adapt to flexibility and last minute changes in the day.

You will care for adults in their own homes to provide practical, physical, emotional, and social support to service users to achieve their desired outcomes. What we're looking for:
* Level 3 NVQ: A level 3 NVQ in Health and Social Care is desirable but not essential.
* Experience: As this is a senior role, we are looking for someone with previous experience within the Health and Social Care sector, and a previous senior position is desirable.
* Driving license: You must have a UK driving license and your own car with business insurance.
* Flexibility: You should be comfortable to work a mixture of daytimes, evenings, and alternate weekends.
