The Discharge Liaison Officer is a new and exciting role supporting and facilitating the safe and timely discharge of patients. The key element of the Discharge Liaison Officer role will be to communicate with BTHFT colleagues, the MDT, patient, families, medical and nursing staff to ensure everyone is working to the same discharge plan. This list is not exhaustive and it is expected that there will be other key relationships which will develop within this role. Main duties of the job The post holder will need to be prepared to work 7.5hr shifts 5 days per week. The role will be to support the MAIDT to facilitate effective discharge.
