We are part of Community Care PCN comprised of 3 local practices and our large cohort of additional roles staff support and share the delivery of clinical care ensuring our patients see the right clinician for their needs. Main duties of the job To work as an autonomous practitioner, responsible for the provision of medical services to our practice population of 31,500 patients, delivering an excellent standard of clinical care whilst complying with the GMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with our Practice and PCN multi-disciplinary teams. You will undertake a variety of GP duties including surgery and telephone patient consultations, online, telephone and in person triage of patients, home visits, referrals, test results, prescriptions, dealing with queries, clinical administration, reports and correspondence. You will take responsibility for professional, autonomous decisions in relation to assessing presenting problems, whether self-referred or referred from other healthcare workers. Support screening of patients for disease risk factors and early signs of illness and manage long-term conditions and patients with complex problems. You will have a strong focus on patient care and quality standards, ensuring clear and contemporaneous consultation notes, accurate record coding and diagnosis, and complying with audits for QOF / Enhanced Services etc. About us We are a large, innovative GMS Practice based in Hall Green Birmingham with an increasing patient list of 31,500 patients.
