An exciting opportunity has arisen for a Lead Frailty Nurse Specialist to join our team.
We are looking for an enthusiastic and adaptable nurse to lead on the frailty service on both sites within Maidstone and Tunbridge Wells NHS Trust.
As the Lead Nurse in Frailty, you will use your clinical experience, expertise and advanced skills to support the development of enhanced clinical competencies ensuring appropriate training, support and oversight to maintain a clinically competent workforce.
You will work closely with all relevant clinical teams, providing clinical advice, supervision, mentorship and work-based facilitation. Develop and complete audits as required. Working closely with users of our services and other stakeholders to ensure appropriate and timely service development and improvements to patient care.
Your core working days will be Monday - Friday with notice provided for any weekend work and out-of-hours work if this is required to meet the need of a clinical team.
Main duties of the job
1. Support the development clinical competencies within Frailty, ensuring access to appropriate training to maintain a clinically competent workforce in the provision of Geriatric Medicine .
2. Provide mentorship and work based learning facilitation, in-house curriculum design and teaching in Frailty.
3. Act as an expert clinical advisor providing leadership and professional support for the Acute Frailty teams. Also establish links with the community teams including complex care nurses and Home Treatment Service
4. Keeps accurate, contemporaneous records and written reports as required.
5. Actively participates in frailty meetings within the trust and community cross organisation working
6. Manage and support the development of the outreach team across site
7. To identify and implement a range of clinical and process audits that will develop the Service and ensure these are completed in line with the annual Audit Plan
8. Develop and maintain professional links with service professional leads across the sector and neighbouring organisation's meetings in order to share information, co-work and inform best.

Detailed job description and main responsibilities
Once your pre-employment checks have been completed you will be contacted by Recruitment to arrange your start date which will be in line with one of our Corporate Inductions.
Following your Corporate Induction you will be expected to attend the Trust’s 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. The programme is full-time (please email if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.
Please note that appointment to this post will be subject to a satisfactory Disclosure & Barring Service Check at Enhanced Level. (Enhanced level now includes regulated checks against vulnerable adults and children)

9. Masters or equivalent experience
