Midlothian Older Adult Mental Health Community Teams (Dementia Team) at Bonnyrigg Health Centre is looking for enthusiastic individuals to join our team as Post Diagnostic Support Workers within our Mental Health Older Adult Services. The post holder will deliver a minimum of one year’s Post Diagnostic Support to people newly diagnosed with dementia, providing advice, information, emotional and practical support to individuals and their carers.
The post holder will work within the integrated Older Adults Community Mental Health Team, collaborating with the Community Psychiatric Nursing Team, Occupational Therapy Team, Social Work Team, Dementia Advisor, and other relevant stakeholders.
Key Duties:
1. Act as a named contact for the person and their families/carers.
2. Utilize the 5 Pillar model to work with people newly diagnosed with dementia.
3. Provide information, advice, and support for people who are in the process of, or have been recently diagnosed with dementia, their carers and families.
4. Enable individuals and their families to develop a robust personal plan that utilizes their own natural supports.
5. Ensure that post diagnostic support is personalized and flexible, leading to agreed and specific personal outcomes.
6. Review and monitor change and adapt to the person’s needs.
7. Utilize a health promotion approach to ensure awareness and understanding of the range of diseases known as dementia.
8. Liaise and collaborate with other professionals involved in providing dementia support and care services.
9. Facilitate the establishment of support groups/social groups for patients and/or their carers and families, including short-term educational groups and social groups.
