Job summary An exciting opportunity has arisen for an enthusiastic, experienced Health Care Assistant (or equivalent) to join our Older Persons Team as a Care Coordinator within the Devizes PCN. The role would involve managing your own caseload and working in collaboration with other clinicians in the team to provide support to our housebound and older community. Main duties of the job The care coordinator will play an important role within the PCN, supporting clinicians who work with the housebound frail/elderly and those with long term conditions. You will assess a patient's active daily living needs, supporting patients and their carers to navigate and access additional support services,whilst helping them to manage their own health and wellbeing. This will involve making referrals to community services, social prescribing link workers, health and wellbeing coaches and other health professionals. The care coordinator will contribute to tackling inequalities in health and social care in the housebound and older population. Care coordination can help break the cycle of the inability to cope that may potentially lead to additional health care intervention, hospitalisation and increased social care intervention in the community.
