Main duties of the job We are currently looking for Band 5 Nurses to join the Leeds Memory Services. This is a city wide service with a focus on assess ment and diagnosis of dementia serving to meet the needs of the people of Leedswho are experiencing memory problems however, this position is for south locality. This service providesthe diagnostic element to working with people with dementia and does not include any ongoing carecoordination. If ongoing support is required the post holder will be responsible for all necessary referrals, there is no caseload element to the work.You must be able to demonstrate a passion for working with people with Dementia, be committed and motivated to working with older people and take an active role in service development.
