Social Care Facilitator (Intermediate Care Services) - Ref: SS08124
Grade: G7; Salary Range: £32,654-£37,035 per annum.
This is an exciting time to join Walsall Adult Intermediate Care Services (ICS).
ICS is a multi-disciplinary and multi-agency team operated jointly between Walsall Health Care NHS Trust and Walsall Council Adult Social Care. We are a dynamic, welcoming team working with adults, their carers, and families. The role entails supporting individuals to maximize their full potential and promote their independence following an acute episode of illness or hospital admission. The team supports with hospital discharge, rehabilitation, crisis intervention, and preventing hospital admissions. We are committed to working in collaboration with partner agencies to achieve positive outcomes for individuals.
We adopt a strength-based approach in our practice and outcome-focused support planning. Applicants are required to have experience of safeguarding adults. The Social Care Facilitator role is expected to undertake statutory duties in line with the Care Act and Mental Capacity Act.
We offer regular supervision and peer group support, with a commitment to your ongoing career development and progression. We will ensure you are given the opportunity to access post-qualifying training and ongoing development opportunities. ICS is an innovative service supporting hospital discharge and interim community pathways aligned to the national vision, for which Walsall has been recognized by NHS England as an area of good practice.
ICS is a seven-day service across 365 days a year, and successful candidates will be required to work weekends and bank holidays on a rotational basis.
