We are a friendly, well-established, forward-thinking GP practice seeking a dedicated Practice Nurse to join our busy medical practice in Rhosllanerchrugog due to retirement. The practice currently has 6250 patients, 4 GP Partners, and a fantastic supportive Clinical and Admin Team.
As a Practice Nurse, you will be responsible for providing high-quality and compassionate nursing care to our patients and supporting the wider clinical team.
Main Duties of the Job

1. Diabetes

4. Coronary Heart Disease
5. Anticoagulation Monitoring as well as NOAC and DOAC Therapy
6. Cervical Cytology
7. Childhood Immunisations
8. Travel Immunisations
9. Vaccination Programme Training in Flu and Covid
10. Learning Disabilities
11. Infection prevention and control
About Us
Beech Avenue Practice is a training practice, and we currently welcome GP trainees and medical students several times a year. We are also a research practice and would encourage the successful applicant to get involved so that we can build our portfolio in this area as we believe it has positive benefits for both staff and patients.

Job Responsibilities
Primary Duties & Areas of Responsibility
1. Assess, plan, develop, implement, and evaluate treatment programmes for patients that promote health and well-being.
2. Help the practice develop and deliver specialist primary nurse-led services, including chronic disease management.
3. Manage programmes of care for patients with chronic diseases by planning, providing, and evaluating care under agreed guidelines and patient group directions.
4. Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence-based, cost-effective, individualised patient care.
5. Where necessary, evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated.
6. Make appropriate referrals to other members of the primary, community, and secondary care teams.
7. Undertake regular medication reviews for patients for groups of patients deemed suitable by the clinical team.
8. Administer child and adult immunisations and vaccinations in accordance with national and local programmes.
9. Undertake procedures including: venepuncture, BP management, urinalysis, peak flow, spirometry, stitch/clip removal, cytology, and wound management.
10. Promote health and well-being, giving general education advice on diet, contraception, smoking, chronic disease management, exercise, etc.
11. Initiate and carry out programmes of health screening as agreed with the practice clinical team.
12. Enable, support, and encourage individuals, families, and groups to address issues which affect their health and social well-being.
13. Contribute to the practice achieving its GMS contract obligations including quality, organisational, and access targets.
14. Maintain accurate records according to NMC rules. Ensure all data protection requirements are met when gathering, recording, and storing patient data.
15. Liaise and maintain good working relationships with all members of the practice team.
16. Promote a learning environment for patients, nurses, and other health professionals.
17. Assist with the planning and implementation and teaching for practice staff including medical students and health care assistants.
18. Attend and contribute to relevant internal and external meetings as required.
19. Maintain a personal progress and training plan to keep up-to-date with current thinking and developments.
20. Promote evidence-based practice through the use of the latest research-based guidelines.
21. Monitor the effectiveness of their own clinical practice through quality assurance strategies such as the use of peer audit and review.
22. Maintain professional registration.
23. Work within the latest NMC Code of Professional Conduct.
24. Record accurate consultation data in patients' records in accordance with NMC guidance and other standards.
25. Keep up to date with pertinent health policy and work with the practice team to consider the impact and strategies for implementation.
26. Work collaboratively with colleagues both internally and externally.
27. Identify opportunities to delegate tasks to junior members of the nursing team.
28. Help the practice develop in a cost-effective manner, monitoring and using resources appropriately.
29. Participate in practice meetings as required.
30. Participate in audits and inspections as required.
31. As part of the nursing team, ensure all appropriate practice policies are fully implemented.
32. Promote effective communication, relationships, and team working within the practice.
33. Provide clinical educational advice to support the development of the team and range of services provided.
34. Partake in clinical supervision for own development purposes.
35. Facilitate and promote the use of up-to-date technology to develop practice.
Person Specification
Qualifications
* Adult nursing qualification
* Be subject to an enhanced CRB check, the outcome of which must be satisfactory to the practice.
* Holding or working towards qualifications in family planning, asthma, diabetes, CHD, and cytology.
Experience
