Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a pivotal role in managing patient care, ensuring that individuals receive the necessary support and resources throughout their healthcare journey. This position requires strong organisational and interpersonal skills and the ability to work effectively in an office environment while liaising with healthcare professionals, patients, and their families.
Duties
1. Coordinate and manage patient care plans, ensuring that all necessary services are provided in a timely manner.
2. Serve as the primary point of contact for carers and patients, addressing their inquiries and concerns with empathy and professionalism.
3. Collaborate with healthcare providers to facilitate communication regarding patient needs and treatment options.
4. Maintain accurate and up-to-date records of patient interactions, care plans, and progress notes.
5. Assist in scheduling appointments, follow-ups, and referrals to specialists as needed.
6. Educate patients and their families about available resources, services, and support systems.
7. Monitor patient progress and adjust care plans as necessary to meet evolving needs.
Qualifications
1. Previous experience in a healthcare setting is preferred and strong interpersonal and organisational skills.
2. Excellent communication skills, both verbal and written, with the ability to interact effectively with carers, patients, families and commissioners.
3. Proficient in using office software, e.g., Acces Careplanning & PeoplePlanner.
4. Strong problem-solving abilities and attention to detail.
5. Ability to work independently as well as part of a team in a fast-paced environment.
6. A compassionate approach to patient care with a commitment to improving the lives of others.
7. Ability to assist with out of office phone as needed and help with care.
Job Type: Full-time
Pay: £24,000.00-£28,000.00 per year
