Job summary We are proud to offer the post of Mental Health Wellbeing Practitioners to join our experienced workforce in Hertfordshire. The post holder will have a desire to develop their skills in working with adults with severe and enduring mental illness, some of whom will have not had previous access to psychologically informed interventions. The post-holder will work collaboratively with service users, their families, carers, and multi-disciplinary team members to provide assessment, care planning and coordination of care, supporting collaborative decision-making about care and treatment. The successful candidate will be part of the core team and duties will also include, holding a small caseload, completing Initial Assessments, Care Planning, Risk Assessing, being part of the Duty Team on a rota basis and will contribute to the implementation of the LIPP (Low Intensity Psychosis Pathway) Service Mini Team. The post holder will support the delivery of a set of wellbeing-focused psychologically informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems under the supervision of a clinical/counselling psychologist or CBT therapist, where this is identified in personalised care planning and support.
