Mental Health Practitioner - Memory Assessment Service

* Relevant NMC registration.
* Previous experience in mental health environments (preferred, but not essential).
* Strong communication, organisational, and timekeeping skills.
* A genuine passion for working with older individuals in the community who are facing dementia and mental health challenges.
Main duties of the job
* Provide tailored support and interventions for individuals with dementia and mental health conditions.
* Collaborate within a multi-disciplinary team to deliver holistic care.
* Monitor and review prescribed medications, ensuring adherence to best practices.

Job responsibilities

* Collaborate with service users, carers, and the MDT to deliver high-quality care.
* Support medication concordance and ensure safe prescribing practices.
* Contribute to case management, MDT meetings, and CPD.
* Maintain high standards of documentation/record-keeping.
* Travel independently across the county to meet time-sensitive deadlines. A full UK driving licence and access to a vehicle are essential, as public transport will not meet service needs. Please confirm in your application that you meet these criteria.

Qualifications
* RMN level 1 RMN level 2
* Evidence of professional development
* ENB 998 or other teaching qualification, mentorship.
Experience
* Experience in acute or community mental health setting
* Experience of working with people with a diagnosis of dementia
* Community experience
* Primary care experience
Skills/Abilities
* Able to demonstrate commitment to high quality care and service provision
* Ability to work flexibly as part of a team.
* Able to assess, plan, and implement care
* Teach and support student nurses or OTs and HCAs
* Standard keyboard skills and ability to communicate through IT using packages such as Word.
* Time Management and prioritisation Skills.
* Understanding of clinical supervision
* Accepts responsibility and accountability for own work and can define the responsibilities of others
* Recognises the limits of own authority within the role and seeks and uses professional support appropriately
* Understands the principle of confidentiality
* Responsible for maintaining registration with NMC and or other professional bodies
