A fantastic yet rare opportunity has arisen within the Adult Community Learning Disability team for a Band 6 Community Practitioner to join our team working within our Specialist Behaviour pathway.
A team made up of a wide range of MDT principles with a friendly specialist approach to supporting individuals with a learning disability maintain independence within the community. Overcoming health inequalities and promoting wellbeing our team has the fortunate opportunity to enhance the lives of our patients.
Main duties of the job
To manage case work under direction of care co-ordinator/ Pathway ClinicalLead and provide ongoing Assessment and Recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low, under the supervision of the Pathway Clinical provide, under the direction of a Care Coordinator and or Pathway Clinical Lead, effective treatment and intervention to service users with complex and substantial needs requiring support from our Specialist Behaviour deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence. To provide compassionate care that is based on empathy, kindness, respect and dignity.
Ability to work flexibly to meet the needs of Service Users across pathways which may regularly include working flexibly within service hours. Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

Person Specification
Education and Qualification
Essential
1. Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
2. Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
3. Up-to-date knowledge/experience of relevant legislation
4. Proven track record of post registration clinical experience relevant to the post.
5. Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
Desirable
6. Experience of working within a community team
7. Non-Medical Prescribing
8. Additional training/qualifications in positive behaviour support
Kowledge and Experience
Essential
9. 12 to 24 month band 5 qualified nurse experience dependent upon service need
10. Awareness and understanding of professional accountability, NMC standards and other local and national initiatives
Desirable
11. Understanding/experience working with challenging behaviour/ positive behaviour support
Skills & Competencies

12. Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs
13. Risk Assessment and Risk Management / Safety Planning skills
14. Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in Guidance)
15. Demonstrate effective written and verbal communication skills
