Job Title: Qualified Adult’s Social Worker – Hospital Discharge Team
Location : Brent
Rate : Up to £36.00 per hour
An exciting job role has recently become available in Brent. They are looking for a new member for their Hospital Discharge Team.
As part of our Hospital Discharge Team, you’ll play a crucial role in ensuring safe and effective discharges.
What will your responsibilities be?
You will undertake Care Act 2014 assessments, focusing on the person’s well-being and adopting a person-centered, strengths-based approach. You will be responsible for the team’s more complex work. You will empower individuals by promoting choice, control, and independence through reablement approaches. Additionally, you will work in partnership with other professionals, source provisions, and safeguard vulnerable adults.

Qualifications and Experiences:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
