Job Description:
Liquid Personnel are recruiting a Social Worker for the Preparing for Adulthood Team in Bromley. The role involves assessing individuals, carers, and agencies in accordance with the Care Act (2014), initiating and coordinating care and support plans with service users, carers, professional colleagues, and voluntary/private agencies.
* Ensure the ethos of wellbeing is adhered to when working with vulnerable adults and their carers.
* Participate in duty operations, respond to needs, including safeguarding enquiries and Best Interest Assessments (BIA) when required.
* Provide assessment services to individuals, carers, and other agencies per Care Act (2014) criteria.
* Initiate and coordinate care/support plans for service users and carers, collaborating with colleagues and agencies.
* Support staff as agreed with Line Manager and in line with Council procedures.
* Offer support and advice to service users and carers.
* Assess and prioritize needs, commissioning appropriate services and authorizing expenditure within limits.
* Monitor and review individual care and support plans.
* Maintain client records according to Council policy.
* Respond to complaints within the Council’s procedures.
* Implement new legislation affecting young people as directed.
* Contribute to service development and provide feedback for future services.
* Advise on the quality and value for money of services.
* Participate in multi-disciplinary and inter-agency meetings, representing the team as required.
* Coordinate, participate in, and chair case conferences and safeguarding meetings.
* Work across children and adult services as needed, supporting young people aged 18+ or from age 14 for future planning.
* Perform any other duties necessary for the team/service.
Qualifications:
* For qualified Social Workers, registration with Social Work England.
* Evidence of completing an assessed and supported year in employment (ASYE) or equivalent for recent graduates.
