Job Summary
We are looking for a motivated, enthusiastic person to join the Discharge team working within Buckinghamshire Healthcare Trust. The team supports the discharge of patients with varying needs.
You will work under the supervision of existing discharge coordinators supporting the planning, coordination, and transfer of care of patients with complex discharge needs from the point of admission to safe and effective discharge.

Main Duties of the Job
* To support the delivery of high-quality discharge and patient flow for patients with complex discharge planning needs, acting as the patient's advocate when necessary.
* To liaise with ward staff to identify patients with complex and multi-discharge planning needs that require intervention and work with the complex discharge coordinators to ensure all appropriate assessments/referrals are made and the patient's needs are met in a timely manner to prevent delays.
* Communicate and liaise with patients, relatives, carers, and staff in community, social services, and voluntary agencies to develop an optimum plan for the patient from the point of admission to discharge.
