Southend East PCN is looking for a Health & Wellbeing Coach to join our Personalised Care Team within the PCN. The Health Coach will work with our already established team of 3 social prescribers and 3 Care Coordinators to provide patients with a holistic approach to healthcare. Southend East PCN has its very own PCN building - Norton Place, which it operates from based in Shoeburyness.
Main duties of the job
* Provide capacity in the PCN for weight management programmes for its member practices.
* Utilise Systmone to book and manage your own workload.
* Utilise Systmone for the PCN to increase coverage of observations such as Blood Pressure checks, Smoking Status etc.
* Work with the Digital Transformation Lead to provide outreach and communal events within our locality.
* Be a part of the PCN INT model, an MDT approach with all professionals in the INT.
* Utilise Windows based applications such as MS office (Powerpoint, Excel, Word)
* Contribute as a team member to supporting other staff employed in the PCN to carry out the delivery of the objectives using data.
* Be able to fully utilise the wider PCN team, and have good pathways to book in with a wide range of healthcare professionals.
* Manage and prioritise a caseload, in accordance with the health and wellbeing needs of their population. Supporting practices teams in identifying patients who will benefit from the support and encouragement you can offer.
* Provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently. Helping them understand what is important to them and working with them to get there.
About us
Southend East PCN is located at Norton Place in Shoebury, along Ness Road. We have our own PCN building which is extremely active and is in use by our 5 member practices in the local area. We cover approx 37,000 patients and offer a range of appointments from our wide range of staff, fully utilising the ARRS budget. Our team consists of pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists and assistant practitioners. We have 2 clinical directors who are partners in two local surgeries, and an operations manager to provide support wherever needed. We have a strong digital presence and have employed a Digital Transformational Lead, to help our PCN network with other businesses and mature over time.

We are looking for a Health& Wellbeing Coach, who is a positive team player who strives to go the extra mile to help out colleagues and others who come into contact with the organisation. You really want to make a difference! We need someone who can build strong relationships with patients, team members, voluntary sector and is committed to meeting deadlines.
The role will require coaching skills, patience, empathy and a holistic approach to support patients in reaching their health goals. We are looking to appoint an individual who is self-motivated, organised and reliable you do what you say you are going to do!
You will perform a range of administrative tasks, as well as clinical input. Communication with patients, their family members and various Healthcare Professionals.
The successful applicant will be required to have strong IT skills, attention to detail for data collection and good time management to manage their own task list.
Main Duties and Responsibilities

* Introducing patient support such as self-management education and peer support like group consultations.
* Supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.
* Working in with the Social Prescribing Link Workers, Care Coordinators, Pharmacists clinical and non-clinical teams within the PCN to connect patients to community-based activities which support them and help them to take control of their health and wellbeing.
* Explore and support patient access to a personal health budget, where appropriate, for their care and support.
Data Collection
* Set up and run SystmOne searches, under the instruction of the Primary Care Network Operations Manager, Public Health and the Clinical Directors, to identify the PCN needs and also to monitor progress against indicators.
* Collect data and review the digital triage system in accordance with PCN needs.
* Retrieve data from information systems and clinical documentation for the purpose of undertaking clinical audit (consistent with the audit measures and data collection instructions) or support clinical staff carrying out these functions directly.
* Set up databases and spreadsheets for the collection of audit data and enter audit data as required.
* Check the accuracy, completeness and timeliness of data entered into a database or spreadsheet for a clinical audit and advise relevant people of missing or inaccurate data or an impending deadline for submission of complete and accurate data.
* Analyse or assist clinical staff in analysing audit data appropriately so that the level of compliance with best practice can be determined and the results used to prompt changes in practice as necessary.
* Contribute to carrying out work needed to ensure that the PCN meets external requirements related to clinical audit.
* Maintain documentation related to clinical audit consistent with approved policies, the Data Protection Act, General Protection Data Regulations (GDPR) and Human Rights Act and ensure that the information is available for reporting purposes.
* Record progress of clinical audit activity on the PCN database systems ensuring that there is a detailed, timely and accurate record of all allocated projects. This will include evidence of improvements achieved through clinical audit.
* Contribute to supporting the smooth running of the Primary Care Network.
* Attend meetings as required and any other local or regional audit meetings pertaining to Capacity and Access as requested by the PCN Manager or Clinical Director.
* Attend meetings with the other PCN Clinical Audit Team for the purpose of reviewing the departmental activity and sharing general information.

ARRS Requirements
* B4.1. Where a PCN employs or engages a Health and Wellbeing Coach under the Additional Roles Reimbursement Scheme, the PCN must ensure that the Health and Wellbeing Coach:
* a. is enrolled in, undertaking or qualified from Personalised Care Institute accredited health coaching training as set out in the NHS England Workforce Development Framework, including training, or apprenticeships to obtain a level three occupational standard, with the training delivered by a training organisation listed by the Personalised Care Institute;
* b. adheres to scope of practice and professional standards set out in the workforce Development Framework for Health and Wellbeing Coaches;
* c. has formal individual and group coaching supervision which must come from a suitably qualified or experienced individual; and
* d. working closely in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider to identify and work alongside people who may need additional support, but are not yet ready to benefit fully from social prescribing.
