Participate in MDT virtual care home ward rounds, proactively identifying patients to be assessed in conjunction with care home staff. Work as part of the MDT to optimise patient care and refer to or liaise with other keyservices such as the dietetics team, Specialist care home nurse, district nursing teams andthe palliative care team. Attend any training courses which are identified as being key to current practice.
