Responsibilities
Conduct comprehensive assessments to identify client needs and strengths.
Develop and implement individualized intervention plans based on client assessments.
Provide counseling and support to clients and their families to address social, emotional, and financial challenges.
Advocate for clients to access necessary resources such as housing, healthcare, and financial assistance.
Monitor client progress and adjust intervention plans as needed to ensure effectiveness.
Collaborate with other professionals and organizations in the community to coordinate services for clients.
Maintain accurate and up-to-date client records and documentation in compliance with organizational policies and legal requirements.
Requirements
Bachelor's degree in Social Work (BSW) or related field; a Master's degree in Social Work (MSW) preferred.
Current licensure as a social worker in accordance with state regulations.
Proven experience in case management and client support services.
Strong understanding of social services and community resources.
Excellent communication and interpersonal skills for effective client interaction.
Ability to work independently as well as collaboratively within a multidisciplinary team.
Strong problem-solving skills and the ability to manage challenging situations with empathy and professionalism.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £38.54
Location: Plymouth, South West England
