Social Worker - Reablement Team

Full time: 36 hrs a week (Mon - Fri)

Hatched Recruitment is looking for a Social Worker in the Islington area. The post holder will be part of the Reablement Service - In-house provider services. Islington's Reablement service provides a short intervention for Islington residents of up to six weeks with a view to supporting people to become more independent.
The Reablement Social Worker will be part of a multi-disciplinary team comprised of team leaders, home support workers, and case managers, working together with Pharmacists and Therapists to improve the independence of people undergoing Reablement in Islington.
The role of the Reablement Social Worker includes:
1. Guiding individuals with complex needs through the up to six-week intervention.
2. Coordinating a person's Reablement journey by drawing on the expertise of team members to maximize independence and meet Reablement goals.
3. Completing social work specific activities and assessments in line with Safeguarding Adults procedures and the Mental Capacity Act.
4. Reporting to the Reablement Manager for all operational activities and receiving formal supervision from the Senior Practitioner Social Worker.
5. Working in a multi-agency context, sharing information, conducting joint assessments, and care planning to provide a holistic understanding of needs and services.
6. Promoting and safeguarding the welfare of vulnerable adults in your care and those you come into contact with.
7. Carrying a caseload of varied complexity and taking on care coordination responsibilities.
8. Undertaking Mental Capacity Assessments in accordance with the Mental Capacity Act 2005.
9. Ensuring that the Care Act 2014 is implemented in all aspects of the role.
10. Working with service users, carers, and other professionals to minimize and manage risk while enabling service users to maintain their chosen lifestyle.
11. Obtaining informed consent for all care-coordinated cases.
12. Keeping the Team Manager informed of any significant events relating to the service.
13. Providing advice and consultancy to colleagues within the team and a wider range of stakeholders, including the NHS, the Independent sector, and members of the public as appropriate.
14. Maintaining records of work undertaken and entering data on the database in accordance with Council Policy.
Flexible working over a 7-day working week will be required to fit in with the needs of the service.
