Surrey and Borders Partnership are seeking to recruit a qualified OT or Nurse to work as part of our Integrated Care Team within primary care, supporting people 18+ with long-term physical and mental health issues to maximize their potential and well-being.
Based within the North East Hants area, the successful candidate will have a minimum of two years' experience working within community mental health services and possess an ability to work autonomously within a primary care community setting.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
Main duties of the job
Working within Primary Care, you will be able to facilitate mental health/cognitive assessments and formulate diagnosis, treatment/post diagnostic plans with the supervising Consultant Psychiatrist.
The successful candidate will favour a person-centred, psychosocial approach to practice, enabling independence and avoiding hospital admissions where possible.
Excellent risk assessment, communication and documentation are an essential skill set for candidates, along with a willingness to problem solve and signpost to other services, where appropriate.

* Professional Qualification in Mental Health Nursing or Occupational Therapy
* Non-medical prescribing
Experience
* Experience of providing brief psychosocial interventions with adult/OA mental health patients
* Experience of conducting MH assessment/Memory assessments
* At least two years experience working in a community setting

Salary: £48,270 to £54,931 a year (Incl. 5% Fringe HCAS) per annum, pro rata.
