Senior Digital Health Assessor
Band 6 - 37.5 hours - Full Time, Permanent
We are looking for Individual Nurses and/or Paramedics with sound clinical knowledge and experience in an acute setting/background working with Adults. You will have excellent assessment skills demonstrating a logical decision-making process that ensures patient safety.
This post will require you to utilise your clinical assessment and management skills to enhance the experience of all appropriate patients under our care to access and use the facilities/services available via Digital Consultation & telephone triage assessments.
This post will enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills, and support the technological advances being developed at Tameside and Glossop Integrated Care NHS Foundation Trust.
The Senior Clinical Assessor will support and participate in the development of integrated pathways and care packages that enable patients to be safely cared for outside of the acute hospital setting and support palliative/end of life patients through a designated telephone service.
We provide our hospital and community with a Clinical assessment service which incorporates:
1. Video assessments
2. 111 calls for adults & children (Training is provided)
3. Telehealth Service
4. Virtual Wards
5. ED streaming (Based in ED)
6. NWAS Response & crisis response service.
Main duties of the job
The Digital Health Service is currently being developed to support Tameside and Glossop's new Model of Care. This post provides the opportunity to lead the way in digital health in Tameside and Glossop and be part of a new and emerging team.

This post will enable Adults & Children to be cared for safely by remotely supporting established triage assessments & care packages at home or in their place of residence.

Essential
* Extensive clinical knowledge in the care and management of the acutely ill/complex care/palliative care/intermediate care patient within and outside of hospital.
* Proven track record in acute care supporting Adults & Children or pediatric background.
* Evidence of personal development.
* Knowledge of Risk Management.
Desirable
* Previous experience of remote video assessment or telephone triage service.
* Experience of managing and delivering sustained change.
* Experience of working with Care Home Teams.
Qualifications
Essential
* RGN with current NMC registration.
* ENB 998.
* Evidence of assessment & Triage of Children.
* Significant experience in managing acute conditions.
* Ability to travel to various locations between Tameside and Glossop.
Desirable
* Working towards a degree in a relevant healthcare topic.
