We are looking for a highly motivated individual, with experience of working in primary care to work alongside our existing Nursing team. Chronic Disease Management with a knowledge of QOF and Primary Care experience is Desirable. Working hours are 37.5 per week over 4 or 5 days. Working days will include Monday's and Friday's.
