This post is part-time working 2-3 days per week.
We are committed to safe and manageable workloads for our clinical team offering generous appointment times and daily clinical huddle. An exciting opportunity has arisen for a Specialist Practice Nurse role which is an integral part of the General Practice team and is responsible for providing nursing treatments to patients in liaison with GPs, ACPs or independently according to agreed protocols. The Practice Nurse will be professionally accountable to patients under the NMC.
The Practice will support the continuing education and professional development of nurses in the form of attendance at study days, courses and with in-house training, as deemed appropriate for the benefit of both the individual and the Practice.
It will be necessary to hold a full driving licence and to have own transport to travel between the practice sites.
In addition to the primary responsibilities, the Specialist Practice Nurse will:
1. Support and / or lead a named area of clinical compliance in line with Practice need
2. Utilise their chronic disease knowledge and skills to deliver care to patients in General Practice providing support for both patients and colleagues in recognised quality and outcome chronic disease areas.

Job responsibilities:
In addition to the Practice Nurse role, the Specialist Practice Nurse will be required:
1. To use EMIS clinical system for recording consultations and for data capture.
2. To liaise with other primary healthcare team members.
3. To play an active role in the continuing development of the practice and support practice management plans.
4. To act in a nurse leadership role for specific chronic disease areas under the Quality and Outcomes Framework of the GMS contract by pro-actively aiming to meet these targets
5. To actively support the development of chronic disease systems and processes across sites in liaison with ANP and GP Leads
6. To provide holistic, high quality, clinical care working always in accordance with NMC professional guidelines and scope of competence.
7. To be involved in the management of patients with chronic disease and evaluating their care where indicated using appropriate recall systems to actively signpost the patient to the correct professional
8. Partake in research and audit as appropriate re: named chronic disease area
9. To provide subject matter expertise on specific chronic disease matters as part of the practice quality improvement work in liaison with the clinical team
10. To attend and input to the regular practice meetings, as deemed appropriate.
11. Attendance at regular training / updates / meetings as identified in your Personal Development Plan
12. Attend clinical supervision in line with professional and CQC requirements.
13. To maintain skills and develop competence in new areas depending on service needs.

Person Specification:
Qualifications Essential:
* NMC Registered Nurse
* Diploma Level or equivalent in Chronic Disease
Desirable:
* Educated to Degree level
* Independent Prescriber qualification
