Job summary The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and technicians in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them. The post holder will be supported to proactively manage patients within their scope of practice and those with complex polypharmacy with a focus on structured medication reviews in an area of interest. The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services under DES specification. The post holder will support integration between primary care, community and hospital pharmacy to improve patient outcomes. The post holder will be supported to develop their role to become a nonmedical prescriber. Main duties of the job We are looking to recruit a highly motivated and enthusiastic Clinical Pharmacist to work alongside and provide complementary skills to our existing clinical teams whilst building strong effective working relationships with local community and ICB Medicines Optimisation Team to improve care and services to our patients. The successful applicants will have good clinical backgrounds and possess the requisite skills required to support safe, evidence based, clinically and cost-effective prescribing and provide support to general practice clinicians. Experience in general practice is desirable but not essential and qualification as an independent prescriber would be an advantage.
