The Medicines and Pharmacy Team at The Roxton Practice are an integral part of the team, using and sharing pharmacy expertise to support the practice in effective medicines management and optimal patient care. Main duties of the job Clinical Pharmacy in General Practice is part of a transformation program to develop a new model of care that aims to deliver person-centered, coordinated care across the district. The model is supported by national policies like the Five Year Forward View and GP Forward View. The goal is to utilize the role of pharmacy within primary care to help patients stay safe and well, reducing hospital admissions and demands on general practice. Pharmacy technicians pre-registration trainees (PTPT`s) complement pharmacy technician in primary care, clinical pharmacists, community pharmacists, and other members of the PCN multi-disciplinary team. They work towards a qualification as Pharmacy Technician; the difference between qualified pharmacy technicians and clinical pharmacists is the fact the pharmacy techs cannot prescribe or make clinical decisions.
