Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across Warrington.
The Clinical Pharmacy in General Practice model is supported by the direction of national policy the NHS Long Term Plan where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
Clinical Pharmacists play an important role within Primary Care Networks (PCN), working alongside pharmacy technicians, community pharmacists and other members of the PCNs multi-disciplinary team. You will directly contribute to improving patient care and supporting the efficient running of a new model of general practice. You will become the practice expert in medicines optimisation and have input into the following areas but not limited to:
* Undertaking structured medication reviews
* Reconciling medications at discharge from hospital
* Improving medicines optimisation and safety
* Running practice clinics
Main duties of the job
To improve patients' health outcomes and the efficiency of the Primary Care team by providing direct, accessible and timely Clinical Medicines expertise, by working as part of a Multi-Disciplinary Practice Team.
To pro-actively transfer workload relating to medicines optimisation issues from Practice clinical staff to improve patient care, safety, thus freeing up Practice clinical staff to spend more time on other clinical care.
To promote safe, evidence-based, cost-effective prescribing in line with NHS Cheshire & Merseyside requirements.
To provide expert pharmaceutical advice to Practices and the Medicines Optimisation Team.
To provide prescribing support to the designated Practices and team members in areas such as:
* Managing and reconciling medications from Hospital discharge letters
* Medication reviews/structured medication reviews and follow up both over the phone and face to face
* Implementation of prescribing changes
* Review of repeat prescribing
* Audit of practice prescribing
The post holder will provide leadership on quality improvement and clinical audit, as well as managing some aspects of the Investment Impact Fund (IIF).
Experience is preferred but all applications will be considered as training is provided to all new starters and you will be enrolled on to an 18-month primary pathway.

South Warrington PCN is one of five PCNs within Warrington, Cheshire. We provide primary care services to 51,763 patients, across five GP practices. The PCN has recruited staff under the ARRS to support and broaden our primary care team which allows us to provide better care for our patients.

Person Specification
Qualifications
* Independent prescriber or is currently working towards gaining an independent prescribing qualification.
* Clinical Diploma.
Experience
* Minimum of 2 years post qualification experience.
Knowledge and Skills
* An appreciation of the nature of GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Excellent interpersonal, influencing and negotiating skills.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
* Able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
* Able to obtain and analyse complex technical information.
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
* Able to work under pressure and to meet deadlines.
* Produce timely and informative reports.
* Work effectively independently and as a team member.
* Demonstrate accountability for delivering professional expertise and direct service provision.
* Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.

Depending on experience £48,526 to £52,809 per annum pro-rata.
