An exciting opportunity has presented for a motivated and enthusiastic Care Coordinator to work within a community mental health setting alongside our primary care colleagues.
JOB SUMMARY:
To act as care co-ordinator / lead professional within the CPA process for service-users in North East Lincolnshire. This involves being responsible for assessment, developing, implementing and evaluating care plans using a person-centred approach.
Our ideal candidate would be a committed team player as well as being able to work independently demonstrating initiative and passion in promoting good quality care to maximise independence. Within this role, you will gain training, support, and guidance to ensure you are working collaboratively with external agencies to give the best possible care for our service users.
Main duties of the job
This role requires someone who can manage in a fast-paced environment completing assessments to support diagnosis. The day-to-day work for this role will be led by the needs of the service users and the service, which will require effective time management and prioritisation skills as well as effective communication and autonomous working.
All of our staff are an integral part of the multi-disciplinary team within mental health services. The successful candidate will need to understand the importance of engaging with external agencies within our area in order for our service users to achieve high-quality care that we in Navigo always strive to deliver. This should involve using supervision and reflecting on practice to develop new and improved ways of working.
Job responsibilities
MAIN DUTIES RESPONSIBILITIES:
1. Communication & relationship skills
2. The post-holder will work with all members of staff within PCN's and Navigo.
3. They will work in close liaison with health and social care professionals in statutory and non-statutory agencies to facilitate and deliver a range of services to ensure that continuing care needs of service users are met.
4. Participate in multidisciplinary meetings, case conferences and any other meetings as necessary.
5. Effectively communicate complex and/or sensitive information where there are barriers to understanding.
6. Maintain accurate clinical records according to professional standards.
7. Participate in Practice and Management supervision.
8. Promote and implement creative ways of working with service users.
9. Instigate, develop and maintain a relationship with the service user and/or carer to facilitate the accurate delivery of care.
10. Manage highly confidential information in respect of members of the public in accordance with local and national policy.
11. Act as named worker responsible for specific interventions.
Knowledge, training and experience:
1. Attend appropriate training as identified with supervisor through supervision.
2. Maintain and update skills/knowledge relevant to the post and ensure practice is evidence-based.
3. Attend and participate in Team and clinical meetings as required.
4. Participate in clinical supervision process, in appraisal and individual performance review process.
5. Participate in the training, education and induction of new Team members.
6. Have specialist knowledge and training, gained through formal training and experience, relating to older people and mental health.
7. Provide advice and support to other agencies around issues relating to Mental Health.
8. Under supervision make decisions about the range of options for treatment and care plans based on judgement of service-users need.
Responsibilities for patient/service user care:
1. Participate in programmes of care for service users and act as associate worker for service users on CPA and non-CPA.
2. Review service users clinical risk on each visit, reporting back any areas of concern/change in presentation.
3. Provide advice to service users, their carers, relatives and other professionals on needs relating their difficulties and mental health.
4. Initiate individual carer support where required.
5. Participate in clinical governance activities.
Person Specification
Experience
* Experience of working with all age functioning adults.
* Experience of working within a CPA framework.
* Experience of liaising with other teams or services.
* Worked as key-worker/care co-ordinator or equivalent.
Additional Criteria
* Knowledge of assessment, planning, implementation and evaluation of individualised care plans.
* Must hold a full driving licence and have access to a car for work purposes.
Skills & Attributes
* Able to organise and prioritise own workload.
* Good written and verbal communication skills.
* Evidence of engaging in learning activities and professional development.
Qualifications
* Current Registration on part 3 or 13 of NMC register.
* Occupational Therapist or Social Worker currently registered.
