This is a brand new role, being rolled out as part of a pilot project! As such, this is a fixed term contract, up until the 31st March 2026. Should the project be extended, we would review the fixed term contract accordingly. The successful applicant will be responsible for the assessment and specialist employment advise, support and interventions for patients. You will manage a caseload of patients, assessing individual barriers into employment, such as physical health, mental health and social situations. Using the Work Well pathway work plan, the successful applicant will prescriber either multi-disciplinary in-house support or will triage/signpost and refer, as well as collaborating with the care navigation team. The successful applicant will facilitate patients being referred to the service quickly so that they do not remain unsupported whilst on sick leave, and supporting them to return to work swiftly, where it is appropriate, or to ensure a suitable outcome that is best suited to the individual circumstances of the patients, which enables them to achieve a positive outcome from the service. The ideal candidate will have a strong background in public health and a passion for promoting wellness and healthy lifestyles. Purpose Work Well is a government funded programme that offers an early intervention work and health support and assessment service, This provides holistic support to overcome health related barriers to employment, and single, joined up gateway to other support services. We want the Work and Health Coach to provide highly effective support to patients and guide them on their journey to returning, remaining or finding new employment whilst also supporting them in improving their health and wellbeing. Core duties and Responsibilities Core duties will include: Managing a caseload of patients who have experienced common mental, physical and social health problems or long term conditions Care out a vocational assessment, to identify vocational needs and develop action plans in agreement with the patient To provide an evidence based, person centred and holistic initial assessment of the barriers the patient is facing To provide a light-touch holistic support for health-related barriers to employment To provide a provision of a single, joined-up view and pathway into local services to tackle specific need To ensure multiple referral pathways into and onward from the Work Well services, across a range of partners to ensure maximum opportunities for patients engaging the the service. Maintain accurate records of patient interactions and progress This is not an exhaustive list, and it is expected that core duties and responsibilities are likely to adapt and shift as the role develops. Person Specification Essential Educated to GCSE or equivalent level An ability to initiate and develop relationships An ability to work independently, reliably and consistently Good report writing skills Good numeracy and information management skills Solid IT skills Confident with using new IT systems Confident in working with disabled people, or those whom have health conditions Ideally to hold one of the following: Degree level or equivalent level of experience; Level 4 NVQ Diploma in Advice and Guidance; or hold a coaching qualification Desirable Previous experience in coaching or counselling is advantageous A genuine passion for helping others improve their health and well-being is crucial A good understanding of health and social care environments and responsibilities within it. Knowledge of primary care Due to additional recruitment we are advertising for 1 WTE position, one Work Well Coach will be located in each of the 3 Primary Care Networks (PCN's) that we support. Previous applicants need not apply. A working day for us is 7.5 hours, with a 30 minute unpaid lunch break. There is some flexibility with what time the working day can start and finish.
