An innovative role has arisen for a senior community Physiotherapist to join our established integrated intermediate care team. The team provides an urgent community response service to avoid unnecessary hospital admissions and a virtual ward to support patients to remain at home wherever possible and facilitate safe hospital discharge.
This is a 24 hour a day 7 days a week service operating 365 days a year therefore the post holder will be expected to work shift patterns to cover the service including Early, Lates or Long Days and have access to own transport.
Main duties of the job
The post holder will:
* Provide a personalised intermediate care service to adults in their own home within Airedale, Wharfedale and Craven areas.
* Perform Physiotherapy assessment of patients with a diverse range of presentations and complicated physical and psychological conditions, to provide a diagnosis, develop and deliver an individualised treatment programme.
* Work in partnership with medical colleagues, multidisciplinary teams (MDT) and carers to set realistic objectives for safe discharge.
* Support the team in its delivery of interventions.

Person specification
Qualifications
* Degree or equivalent pre-registration award in Physiotherapy
* UK registration with the Health Care Professions Council
* Member of the Chartered Society of Physiotherapy
Experience
* Physiotherapy assessment and treatment skills
* Supervisory and leadership skills
* Experience of MDT working
* Car driver and access to a car for work purposes
* Experience of working in the community
