Berkshire Healthcare Community Nursing Services is looking to recruit a Leader with the confidence and passion to drive change and innovation in our Community Nursing team.
We are keen to recruit an experienced Community Specialist Practitioner who can lead an effective team.
This is an interesting and challenging role that requires strong leadership skills to support the team through ongoing service developments.
A sound knowledge of recent changes within the NHS and current professional issues is imperative, and you must be able to evidence excellent clinical and strong leadership skills.
Working hours- Monday - Friday 8:30- 4:30.
Main duties of the job
* The post holder will be a senior and experienced Practitioner working within a Community Nursing Setting. The post holder will be responsible for the clinical leadership, operational management, and clinical development of the Community Nursing teams, including Community Matrons, and case coordination within the community nursing locality.
* The post holder will lead the development of the Community Nurses and Community Matrons and facilitate joint working between them to support the integration agenda and ensure that the service provided is of high quality, responsive to local needs, and demonstrates value for money.
* The post holder will ensure that the services are effective and well-coordinated, meeting the requirements of all relevant service specifications whilst keeping in line with CQC requirements.
* In addition, the post holder will work closely with the Service Lead, Advanced Community Nurse Practitioner, and Head of Community Nursing Services to develop the strategic direction in accordance with changing national guidelines and local agendas.
* The post holder will work in close collaboration with GP’s, other Health Care Professionals/staff, and patients to provide a patient-centred, seamless, integrated approach to care, consistent with the principles of clinical governance.

* Current NMC (Nursing and Midwifery Council) Registration
* Educated to Degree
* Management Qualification or willingness to work toward
* Evidence of comprehensive post-graduate training in the field of Community Nursing

Previous Experience
* Experience of leading and managing a team, including performance management, sickness monitoring, recruitment, skill mix, and staffing rotas
* Extensive post-graduate experience as a practising professional in community services
* Ability to demonstrate significant relevant experience post-qualification working at a senior level in the areas of community services
* Previous experience of effective stakeholder engagement
* Comprehensive range of supervisory experience both clinically and managerially
* Multi-disciplinary team and partnership working
* Coaching and supporting staff
* Working proactively with staff in implementing innovative clinical practice
