Job summary The Guildhall & Barrow Surgery has a fantastic opportunity for an experienced practice nurse to join our team on a part-time basis, for 26 hours per week. Main duties of the job Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running asthma, COPD, hypertension and diabetes clinics and reviews. New patient and health checks. About us The Guildhall & Barrow Surgery is a two site general practice based in Bury St Edmunds, and Barrow Village. We have 6 GP partners, 7 salaried GPs, an advanced nurse practitioner and a clinical pharmacist. We also have a nursing team of four practice nurses, two HCAs and a nursing associate. We are part of the BSE PCN and have additional medicines management team members, paramedics, a health and well-being coach, and a social prescriber available for our use. We currently service just over 13,000 patients and our list sizes are growing rapidly.
