Job summary We are seeking an experienced and dedicated advanced clinical practitioner to join our team. This position is an excellent opportunity for a highly skilled and motivated individual to work remotely, on a full time basis, while making a positive impact on patient care within a supportive, collaborative environment. Main duties of the job Deliver high-standard, independent care, using advanced clinical skills and knowledge. Conduct patient consultations remotely via telephone, video, and Accurx, ensuring timely care and assessment. Assess, diagnose, plan, and evaluate care for acute and long-term conditions, providing expert treatment and follow-up. Prescribe medication safely in line with legislation and practice protocols, monitoring treatment effectiveness and providing patient education on medications. Take responsibility for making autonomous decisions regarding patient care, including the initiation of emergency care when necessary. Work closely with other clinical team members to ensure comprehensive care for patients, referring to specialists when necessary. Support patients in managing long-term conditions and ensure their adherence to treatment plans. Maintain accurate and up-to-date patient records in line with legal and regulatory requirements.
