* To co-ordinate and provide online rehabilitation group support alongside the Post Covid MDT which includes a consultant lead, physiotherapists, occupational therapists and nurses

* To provide close liaison with family, carers and external agencies to ensure safe and effective care
* To provide a point of contact for patients from their initial referral, through their engagement with and up to the completion of their rehabilitation with the Post Covid service

An exciting opportunity has arisen for a skilled and enthusiastic therapy support worker to join the Post Covid team as part of the core MDT providing hybrid virtual and face-to-face rehabilitation at the Royal Devon University Healthcare NHS Foundation Trust.
The post holder is responsible for providing coordination, patient support, and therapy treatment for a specified caseload of patients within the Post COVID services, working particularly with the Post COVID team. This includes a Consultant lead, Physiotherapists, Occupational Therapists, and Nurses. The post holder will receive relevant training to gain knowledge and skills in Post COVID.
Patient management will include assessment and information gathering to identify patient needs; physical and cognitive assessments; coordination and provision of online rehabilitation; development of treatment plans and review of progress; and delivery of care and rehabilitation. The post holder will work alongside the Post COVID team and liaise closely with family, carers, and external agencies to ensure safe and effective care.
This position is based in the community with some face-to-face clinics at The Royal Devon & Exeter Hospital (Wonford + Whipton). The role predominantly involves virtual work and assisting with group sessions. The post will be static within a clinical specialty but may require flexibility to work in other areas as needed.
Working Pattern: 4 days per week.
