Job summary As a Community Children's Nurse the post holder will be responsible for the management of a caseload of children/young people aged 0-18 years assessing, planning and delivering evidence-based nursing care in the community across West Northamptonshire. You will be working collaboratively with the child/young person, their parents/carers and the multi professional team around the family (Including interagency working). Use of enhanced communication skills to promote good working relationships are required in order to facilitate the best outcomes possible for the child/young person. You will deputise for the Lead Nurse in her absence, regularly provide management in the day to day running of the service and provide supervision to junior members of the team, and mentorship to students in training. Our core service hours are 08.00-18.00 Monday - Friday, working contracted hours as a shift pattern of early shifts - 08.00-16.00 and late shifts 10.00-18.00 - 7.5 hrs per shift, across the working week. On occasion, the role requires the ability to work flexibly across a 7 day service, including unsocial hours and on call, when required for End of Life care at home. Main duties of the job To provide clinical expertise by setting standards in the assessment, planning, implementation, and evaluation of patient's clinical care needs using agreed protocols. To undertake lone working across the area of West Northamptonshire, undertaking appropriate risk assessments and adhering to local policy and procedures. To carry out a range of clinical skills eg placing enteral feeding devices, stoma care, administration of intravenous therapies, administration of injections, wound care, CVAD care in the community setting To demonstrate clinical expertise in providing education and support for the children and families in the community setting. To provide holistic care to the child and family, ensuring that timely referrals are made to other services as necessary. Ensure professional, skilled nursing care is provided to patients within the community/home setting and adapted to meet individuals complex needs.
