Role Overview: We are looking for a talented Nurse (equivalent band 6) with experience of complex care in the clients home or ITU. You will have the opportunity to deliver personalised client-centred outcomes ensuring the clients care delivery is maintained at a high standard. You will carry out assessments, write and update care plans and complete clinical competencies. You will engage, collaborate and support the Clinical Lead Nurse, Senior Clinical Lead, RQIA Registered Manager and Head of Clinical Services in providing support to the care staff.

We are looking for an experienced nurse to work with a client in Northern Ireland where you will work within and manage the package day to day and support the care team.
You’ll have the opportunity to:
* Develop individualised care plans in partnership with the client, family and other health care professionals as appropriate, ensuring goals are patient focused, realistic and achievable.
* Monitor client and family centred outcomes, pro-actively using your expertise to guide and implement changes.
* Undertake holistic nursing assessments in partnership with the client, family and carer and involving other statutory/voluntary/third ‘party’ sector providers where necessary.
* Day to day oversight and management of the care delivery team, working with committed carer teams to maintain all required standards.
* Ensure all recording systems and organisational documentation are of a high standard and up to date.
* Maintain and manage client medication stock, audits and evidence.
* Motivate and manage care delivery team, embedding the company's values and behaviours and a culture of 'pride' in their work.
* Manage and support care delivery team through regular supervisions, competency assessments and appraisals.
* Ensure client compliance requirements are met and maintained; supporting action plans for service improvements that have a positive impact on the client, service quality and business development.
* Have a strong commitment to quality standards to ensure compliance to statutory, mandatory and contractual requirements.
* Maintain your own professional skills specific to your client and undertake required training and development for personal professional development.
* Perform in accordance with national standards, follow legislative requirements, relevant regulations and accepted best practice.
* Act as a role model for good clinical and care governance practice.
Working with others:
* Working in collaboration with the Clinical Lead, motivating care delivery team through informal check-ins, formal supervision, and appraisals.
* Notify clinical lead of any training requirements for the team.
* Lead team meetings with care delivery team.
* Development and implementation of client rota’s, liaising with Care Coordination.
* Oversight of team annual leave to ensure continuity of client care.
* Ensuring the care delivery team are appropriately trained, assessed and signed off as competent prior to lone working.
* Be responsible for promoting and protecting the welfare of the client, upholding the rights, choice and dignity of the client..
* Work in cooperation with members of the multidisciplinary teams to maximise opportunities for the client. Identify information, concerns and ideas for improvement and share them with the Clinical Lead Nurse, Senior Clinical Lead and Head of Clinical Services.
A bit about you, skills and experience:
The role would someone looking to blend clinical nursing skills with coaching, training and management of the care delivery team.
* Current working knowledge and experience of Spinal Cord Injury, in the community or ITU environment.
* NMC Adult registered nurse.
* Professional nursing registration with no restrictions.
* Be able to work 40hrs per week.
* Able to work clinical shifts with the client at least 2 days per week providing all elements of care.
* Support training and competencies of the team, be part of contingency plan on non-clinical days.
* Be competent in writing and updating care plans, risk assessments and other clinical documentation.
* The ability to develop effective working relationships with key stakeholders such as; Case Managers, Integrated Care Systems, General Practitioners, Consultants and other Healthcare Professionals within the multi-disciplinary team.
* Passion to deliver outstanding care in a rewarding and sometimes emotionally challenging environment.
* Have a strong commitment to quality standards to ensure compliance to statutory, mandatory and contractual requirements
* Remain calm and professional, handling queries and concerns with expertise, demonstrating a positive, proactive approach.
* Proficient IT skills
