The Limes Surgery, located in Hoddesdon, is excited to offer a fantastic opportunity for an experienced lead practice nurse to join and lead our dedicated nursing team on a full-time basis.
We are looking for a proactive individual to take charge of delivering safe, effective, and patient-centred care to our practice population. You will manage and lead the nursing team, working closely with GPs and practice management to drive key priorities and ensure high-quality service.
Main duties of the job
1. Managing care for patients with long-term conditions.
2. Acting as the clinical lead for the practice nursing team, prioritising patient care.
3. Guiding the team in implementing practice standards and local initiatives.
4. Fostering a learning environment and promoting a no-blame culture.
5. Overseeing the QoF strategy (and other Locally Enhanced Services) for LTC management and ensuring key performance targets are met (immunisations, flu campaigns, etc.).
6. Leading nursing team meetings, representing the nursing agenda, and driving professional growth.
About us
The Limes Surgery is a well-established and highly regarded practice that serves an 8k patient population and is supported by 5 permanent GPs, along with a comprehensive primary care network (PCN) team, including physiotherapists, pharmacists, social prescribers, health coaches, and care coordinators.
We have an active PPG (patient participation group) that meets regularly to discuss practice issues and patient experience to improve the service.

Qualifications
1. Current NMC registration with an active PIN.
Experience
1. Knowledge of clinical governance issues in primary care.
2. Awareness of local and national health policy.
3. Skills in management of staff and teams.
4. Teaching and mentorship experience.
5. Clinical skills - cervical, cytology, immunisation and vaccination, ear care, wound management.
6. Knowledge of public health issues.
7. Experience managing patients with long-term conditions.
