Description
We are adding dynamic people to our growing Independent Living Program!
As a Center for Independent Living (CIL), we are dedicated to the Independent Living Philosophy and are seeking team members who are passionate about working alongside people with disabilities to achieve their goals.
As an Independent Living Coordinator, you will guide individual participants in the identification of their independent living goals and provide resources, training, coaching, and other supports to ensure that they meet their stated goals. You will also assist with creating and building community relationships, organizing workshops, classes, or activities, and inspiring the independence of all program participants, with a focus on providing CDCS support planning, social security applications and appeals, and independent living skills assessment services.
This is a Rochester-based, in-office position that is full time and benefit eligible. Some travel is required.
Essential Job Functions
* Professionally demonstrate SEMCIL core values and adhere to all required program and agency policies and procedures.
* Deliver quality IL services in alignment with the Independent Living Philosophy.
* Assist participants in the development of goal plans and deliver IL services that align with those plans.
* Research, explore, and consider various options to complex problems and present solutions.
* Provide in-depth, informed, and knowledgeable assistance to support participants with understanding and overcoming complex barriers.
* Maintain proficiency in complex area(s) of services provided.
* Timely and accurate completion of all required documentation.
* Complete training and professional development requirements.
* Maintain and build positive, collaborative relationships with peers, community providers, and referral sources.
* Participate in agency led or invited community events, including presenting SEMCIL information in a public setting.
* Professionally represent the organization.
* Communicate and collaborate with fellow team members to ensure responsive, excellent service for participants.
* Willingness to travel throughout area catchment area and work occasional evenings and weekends.
* Adhere to policies, regulations, and confidentiality requirements.
* Regular attendance and other duties as assigned.
