We have an exciting opportunity for a Band 7 CNS to cover maternity leave, to work within our highly skilled, motivated and dynamic team. We aim to make a difference and improve the experience of palliative care patients at various stages of their disease trajectory. The post will be based within the existing inpatient Palliative Care Team. We offer 7 day face to face support to ensure that people with specialist palliative care needs will receive the best quality care. The post holder will act as a specialist resource Trust wide. Using Specialist Palliative skills and knowledge you will manage a caseload of patients and support their significant others and carers through effective, efficient evidence based high quality care.
The delivery of education and training to multi-professional groups within the Trust is an integral part of the role.
The post holder will also contribute to the teams' development, research and governance through the process of audit and benchmarking.

Main duties of the job
1. To effectively assess, plan and provide specialist care and support to people affected by life limiting illnesses, and also to patients who are at the end of their lives.
2. To participate as part of a multi-disciplinary, working collaboratively with colleagues, patients, carers, other health and social care professionals, statutory, voluntary and independent organisations to ensure optimum patient management.
3. To work within statutory legislation and local policies and procedures in relation to discharge planning and palliative care.
4. To work autonomously and manage own caseload.
5. To participate in providing a 7 day face to face service.
6. To act as a resource, educator and advocate for patients, their carers and health care professionals.
7. To participate in education initiatives to all members of staff.
8. To participate in audit.

Qualifications
Essential
* Undertaken education at Masters Level 7
* Evidence of continued professional development
Desirable
* Either NMP or APACs trained and willingness to continue relevant courses at Level 7.
* Post-basic qualification in cancer/palliative care or relevant subject or working towards
* Advanced Communications Skills Training or prepared to work towards
* Clinical Psychology Level 2
Knowledge
Essential
* Knowledge of palliative care symptom management and experience using medications commonly used within palliative care.
Desirable
* Knowledge of treating Oncological Emergencies and complex symptom management.
Experience

* Experience of attending and presenting patients at MDT.
* Experience of working at Senior Level (Band 6) or Band 7
