Brackley Medical Centre is looking for an experienced Advanced Clinical Practitioner (ACP)/Advanced Nurse Practitioner/Advanced Paramedic (qualified prescriber preferred) to work alongside our friendly and experienced team. We are looking for a motivated and skilled person who loves the challenge of working in General Practice. The role is for circa 26.5 hours Wednesday to Friday (working hours could be flexible).
Join our excellent team of ACPs responsible for urgent appointments and triaging online requests. We also have clinics for our ACP's supporting their interest in specific areas e.g. Cytology and Respiratory.
Main duties of the job
Working as part of a team, you will be providing an efficient integrated same day service to our patients. Working independently through mainly face-to-face consultations, you will be accountable for clinical decision-making, delivering safe and expert care. Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking, clinical assessment, diagnosis, treatment, supply of medication, and evaluation of their care.
The team provides a high standard of patient care for on-the-day appointments for our growing patient list; in addition, undertake home visits, Care Home Rounds, and Hospital Rounds for the Community Hospital situated on the second floor of our building.

We are a 16k patient GP surgery in a purpose-built building which is open Monday to Friday 0900-1830 hrs and Saturday 0815-1215 hrs. The site provides staff and patients with outstanding facilities, with easy access and free parking. We have a large and diverse clinical team of GPs, ACPs, Practice Nurses, and HCAs, along with experienced Patient Services and Administration Teams.
We are a member of the Brackley and Towcester Primary Care Network and have Clinical Pharmacists, First Contact Physiotherapists, Social Prescribers, and Mental Health Workers as part of our team.

Job responsibilities
Roles and responsibilities:
* Ability to independently deliver care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
* Deliver safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the General Practice.
* To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence.
Person Specification
Qualifications
* Registered with the NMC/HCPC
* Non-medical Prescriber
* Completed relevant modules at Level 7
Knowledge and skills
* 2 years experience of working at nurse practitioner or specialist paramedic level within the Primary Care or urgent care setting
Experience
* Experience in the assessment and management of minor injury and illness presentations
* Ability to work independently
* Experience of working in Primary Care
