Main responsibilities: Management of OT service across three rural practices Management and supervision of OTA (experience of this essential) Collaborative working with local services: ICRT, Social Services, Housing Associations, CMHT, acute hospitals, wheelchair services, local hospices, specialist seating providers General practice knowledge wide patient demographic ranging from paediatric to geriatric Wide scope of OT intervention: ranging from provision of basic assistive equipment to more complex case management both long term and short term Person centred approach at all times Integration of wider PCN clinical and non-clinical services to include frailty team and paramedics, as well as social prescribers and care coordinators Offering and facilitating social support within three surgeries (attending coffee mornings and offering advice and support for carers/patients/family members etc) Attending weekly surgery meetings for MDT approach Working and assisting local hospices and palliative services to help facilitate EoL care Mental health service offered on a basic level Ongoing communication and collaborative working with GP and nurse teams within the surgeries
