To support the Lead Consultant in Heart Failure and Cardiology Team in the development and delivery of Heart Failure services at UHNM To be an expert and well-established Heart Failure Clinical Nurse Specialist Provide strategic direction, leadership and co-ordination in relation to the development of heart failure services which are consistent with the Directorate and Division's strategic aims and in line with local and national developments., Lead the development of heart failure specialist nurses and nursing practice. Provide and receives highly complex and sensitive information where there are barriers to understanding e.g. communications to patients and their families on complex and sensitive information on Heart Failure. Highly developed specialist knowledge underpinned by theory and experience on Heart Failure. Highly complex facts that requires interpretation and a comparison over a range of options e.g. assessing patients Plans and organises complex activities with the nursing team with multiple activities and treatments which can require adjustments. Dexterity and accuracy required for clinical procedures such as venepuncture. Organise the clinical management of patients, utilising clinical knowledge and advanced skills to assess, diagnose, plan and implement and initiate changes as required. * Provide strategic direction, leadership and co-ordination in relation to the development of heart failure services which are consistent with the Directorate and Division's strategic aims and in line with local and national developments.
* Develop and support the mechanisms and audits used to evaluate and monitor heart failure services.
* Draw upon advanced knowledge and skills to deliver high quality clinical care to patients/clients and communities. Lead heart failure clinics.
* Provide highly specialist advice, counselling and support to patients, carers and other health care professionals across UHNM and ICB's, with regard to heart failure.
* Be responsible for education and training of heart failure specialist nurses across provide professional leadership and support.
* Lead practice, develop and implement strategies to provide heart failure education and training to healthcare professionals working within UHNM and Partnering ICB and MPFT
* Act as first line manager for the Heart Failure Specialist Nurse team, act as a leader and role model to create a positive and inclusive culture and support effective communication between the Heart Failure team and Directorate Management Team.
* Engage proactively in the Directorate and Division's Clinical Governance processes, including reporting and investigation of incidents and complaints and implementation of relevant action plans.
* Act in accordance with the NMC Code of Professional Conduct.
* Have a responsibility and legal obligation to ensure that information processed for both patients and staff is kept accurate, confidential, and secure in line with the Data Protection Act (1998) and the Security and Confidentiality Policy.

* Work closely with other professionals to provide a multidisciplinary service, in both primary and secondary care settings, liaising closely with the cardiology team.
* Implement standards and audit systems to demonstrate the effectiveness of the service.
* Lead the development and review of policies, protocols, guidelines and practices in relation to heart failure.
* To adjust medicines and advise within scope of NMP qualification.
* Demonstrate an understanding of the ethical, legal and professional implications involved in changing practice.
* Maintain accurate records and develop management plans for patients across primary and secondary care.
* Contribute to/ organise local and regional heart failure groups.
* To work closely with Consultant Cardiologists in the development of heart failure services.
* Plan supportive services for patients suffering advanced heart failure.
* Maintain a Personal Professional Portfolio for NMC revalidation.
* Maintain good relationships, communications with all disciplines within the MPFT and externally.
* Hold non-medical prescribing certificate.
