The Ross-On-wye District Nursing Team are a very supportive and dedicated team, providing care and support to patients in their own homes. We work closely with a wide range of multi-disciplinary teams, including GPs, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provides a wound clinic service based at the Community Hospital, where you will be fully supported to learn and develop your knowledge.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. While undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions. Due to the nature of this role, the ability to meet the travel requirements of the post is essential. Main duties of this role include, but are not limited to:
1. Wound care
2. Palliative care
3. Managing end of life symptoms and providing support for relatives/carers
4. Managing long term conditions in a community environment
5. CVAD and drain management
6. Chemotherapy disconnect
7. Holistic assessments
8. Post operative care following discharge
9. Wound clinic service
10. Venous blood samples
