Job summary An exciting opportunity has arisen in The Phoenix Team which is a midwifery team that specialises in providing specialist care for Vulnerable women. This secondment in the team is specifically focused on young parents and the postholder will work alongside the Young Parents lead midwife .The successful applicant will be will be responsible for a cohort of Young Mothers aged 19 and under with specific focus on Mothers aged 17 and under and will be delivering individualised antenatal and Post-natal Midwifery Care supporting Young women and families with a variety of vulnerabilities throughout their pregnancy. Main duties of the job providing antenatal and postnatal care to young parents providing and promoting parent education working with multi discliplinary teams and agencies working in collaboration with local safeguarding teams making appropriate referrals to other agencies providing and promoting health education Act as the Named Midwife for a cohort of young Work in partnership with other professional colleagues and wider Multiagency partners to care for women with complex needs. Plan and deliver evidence based midwifery care in partnership with the women to provide a supportive relationship, acting as an advocate for women and their families at all times. Monitor maternal and fetal well-being in pregnancy. To monitor and maintain standards of care whilst providing ante natal and post-natal care at venues for example, women's homes, health centre, family practitioner surgery or the hospital.
