The Role:
1. We have two exciting openings for experienced Social Workers specializing in Mental Health and Learning Disabilities within Adult Social Care.
2. This role involves collaboration with Adult Social Care Commissioners to establish a Social Care Mental Health Recovery Pathway and a Supported Living Project.
3. Your expertise will be crucial in defining current service provision and supporting transitions to more independent living arrangements.
What you will do:
1. Provide essential social work input and expertise to the Pathway Planning Team.
2. Carry out needs assessments and review support plans, ensuring alignment with The Care Act, Continuing Health Care, and section 117 Aftercare provision (MHA).
3. Develop and support recovery pathways for individuals with mental health conditions to help them achieve greater independence.
4. Conduct comprehensive reviews of current service packages to identify opportunities for individuals to transition to more suitable community living.
5. Assist in the re-modelling, de-registration, and de-commissioning of services to meet the evolving needs of service users.
