We are seeking a Care Coordinator to join our team and play a vital role in delivering high-quality domiciliary care services. As a Care Coordinator, you will be the main point of contact for both staff and clients, ensuring seamless care delivery that meets the unique needs of each individual. Key Responsibilities Care Planning: Develop, coordinate, and manage individualised care plans to ensure person-centred support that reflects the needs, preferences, and goals of individuals. Scheduling: Organise and allocate care workers to meet client requirements efficiently, ensuring continuity of care and adherence to agreed schedules. Communication: Act as the first point of contact for clients, families, and care staff, addressing concerns, queries, and changes promptly and professionally. Staff Supervision: Provide leadership, guidance, and ongoing support to care workers, including conducting supervisions, appraisals, and spot checks. Compliance: Ensure all care services comply with regulatory standards and company policies, maintaining accurate records and documentation. Problem Solving: Handle emergencies and last-minute changes to care plans or rotas effectively, ensuring minimal disruption to care delivery. Training and Development: Identify training needs and work with the management team to ensure staff are equipped with the skills and knowledge needed to deliver exceptional care. On call duties : to manage the on call phone answering queries. What Were Looking For We are seeking a Care Coordinator with: Experience in a similar role within domiciliary care or health and social care. Excellent organisational and time-management skills, with the ability to manage multiple tasks under pressure. Strong interpersonal and communication skills, with the ability to build positive relationships with clients, families, and staff. A good understanding of person-centred care principles and the ability to apply them in practice. Proficiency in IT systems, including scheduling software and Microsoft Office applications. A Level 3 qualification in Health and Social Care (or equivalent) is desirable. A full UK driving licence and access to a vehicle (if applicable).
