Salary: From £12.00 PLUS paid mileage and paid travel time

Overview:
As a Home Ward Care Professional, you will play a vital role in delivering high-quality Home Care services to patients within the comfort of their homes.
This innovative position means the patient is able to remain at home for their final days and have their wishes met. You will work collaboratively with our multidisciplinary team, Single Point and the Operations Hub to ensure patients receive comprehensive and personalised care.
Responsibilities:
• Provide personal care services, including bathing, dressing, general personal care, toileting, all with empathy and respect for our patients needs.
• Assist with mobility/ transfers to ensure the safety and well-being of our patients.
• Prepare and serve nutritious meals tailored to dietary requirements.
• Mouth care.
• Companionship.
• Administer medications and monitor vital signs as directed by healthcare professionals.
• Perform light housekeeping duties, including cleaning, laundry, and shopping.
• Document client care activities and maintain accurate records of daily interactions.
• Stay updated on the individual care plans and communicate any changes or concerns to the care team.
